Disease management is a system of coordinated heath care interventions and communications for defined patient populations with conditions where self-care efforts can be implemented. Disease management empowers individuals, working with other health care providers to manage their disease and prevent complication.
Disease management may be an effective means to enhance the treatment of individuals suffering from and at risk for cardiovascular disease and stroke by increasing the quality of care, adherence to guidelines and other care protocols, and access to healthcare services.
Essentially, disease management nurses are not trying to manage a patient’s disease for the patient. Rather, their job is to give the patient the tools, education and encouragement to control his or her own disease.
